Re: Personal Information Protection and Electronic Documents Act

As you are aware, federal privacy legislation became effective January I’t,2004. Briefly,
the Personal Information protection and Electronic Documents Act will govem the
collection, use and disclosure of personal information by all Canadian commercial
organizations.


Oakin Insurance Brokers Limited is committed to protecting your privacy and the
confidentiality of your personal information. In order to comply with this new legislation,
we have a designated privacy officer and have developed a privacy policy.
In your dealings with Oakin Insurance Brokers Limited we may have collected some or
all of the personal information from you regarding yourself or other occupants of your
home or people employed in your business:

  • Your name, address, and telephone numbers
  • E-mail address, web site address, and facsimile numbers
  • Information about your existing vehicles, properties, and business
  • Lease Agreements and Financing agreements
  • Names of your lienholder, lessor or mortgage holder
  • Bank account and credit card numbers
  • Occupation and employer
  • Financial and credit information
  • Claims and conviction history including suspensions, lapses, or cancellations
  • Drivers Licence #, dates licenced, class of licence, drivers education certificates
  • Use of vehicles, properties and business
  • Accident and claims history
  • Prior Insurance history including but not limited to the insurance company and policy number
  • Photographs and inspections of you vehicles, properties and business
  • Valuations on vehicles or properties
  • Any other information required to place or maintain the insurance coverage requested as well as assisting you in the settlement of losses

We may use the personal information above to establish and maintain communication
with you, and to provide products and perform services that you may request. These
involve but are not limited to:

  • Obtaining quotes for insurance
  • Placing or maintaining insurance
  • Assisting in claims
  • Maintaining communication with you
  • Advising you on your insurance

We may also disclose your personal information to third parties such as but not limited
to:

  • Insurance companies
  • Financial institutions or financing companies
  • Leasing Companies
  • Landlords
  • Vehicle dealerships
  • Entities requiring confirmation of coverage placed.
  • Lawyers i.e.: for insurance coverage placement

By providing your information to us, you have consented to these uses and disclosures.
You may however withdraw your consent at any time. In some cases refusing to provide
certain personal information or withdrawing consent could mean that we could not
provide you with the requested products, services, or information.
